Coaches,

 Your contribution to our CYO athletic program is as invaluable to us as providing our children with the safest environment in which to play their sports!  It has become an Archdiocesan requirement for coaches to participate in several programs in order to maintain their eligibility to coach CYO athletics.  Please bear with me as I ask you to contribute more of your time than you already do.  Here are the requirements which can be accessed at archdiocese-phl.org/protection, then click on safe environment:

*Keep a copy of all 4 certificates for your own records

 1. Complete a Pennsylvania State Police Request for Criminal Record Check: You can do this online at epatch.state.pa.us/RecordCheckEntry.jsp.  You should receive your certificate instantaneously,  print a copy or save to your computer.  You will need two copies of the certificate.  One copy should be forwarded to the rectory c/o Joan Fogarty or email to her fogie50@yahoo.com , and the second copy is needed for the Child Abuse History Clearance.  This is valid for 5 years.

2. Complete the Pennsylvania Department of Public Welfare Child Abuse History Clearance: Access this form and the instructions at www.dpw.state.pa.us/ServicesPrograms/ChildWelfare/003671038.htm.  Fill out the Pennsylvania Child Abuse History Clearance Form completely.  In the box titled “Purpose of Clearance” check VOLUNTEER.  The form requires that you list EVERYONE you have lived with, and everywhere you have lived since 1975.  Do not forget to include your parents if you lived with them after 1975 (forms have been sent back for not including the complete information).  You will receive your child abuse clearance from Harrisburg usually within 3 weeks.  Please take a copy of the Pa. State Criminal Record Check Certificate and the Pennsylvania Child Abuse Clearance Certificate to Joan at the rectory.  Please keep a copy of these documents, they are valid for 5 years.

3. Attend a Safe Environment Class:  The schedule for the Safe Environment classes can be found on the archdiocesan website:  www.archdiocese-phl.org/protection/SafeEnv/aws_sp05.htm .  You may enroll for a class at  www.virtusonline.org/virtus/reg_list2.cfm?theme=0 .  Please return a copy of the class certification to the rectory c/o Joan Fogarty.  This is required only once.

4. Attend a Coaches’ Orientation Seminar before coaching or take the new online Catholic Coaching Essentials before you coach and attend a Coaches’ Orientation Seminar before your second year of coaching: This is required for any CYO coaches and assistants.  Information can be found on oyya.org/athletics/2009-2010%20Coaches%20Orientation%20Schedule%20for%20Website1.pdf .  This is not required for flag football.
